New EMCDDA review studies the effectiveness of overdose antidote, naloxone
Can naloxone provided in the community help reduce the thousands of drug-induced deaths recorded in Europe every year? This is the question explored by the EMCDDA in a new paper
Understanding the Costs and Savings to Public Services of Different Treatment Pathways for Clients Dependent on Opiates
This paper presents a summary of analysis that considered the costs and savings to public services that might be generated from those receiving treatment for opiate dependence (Department for Work and Pensions, UK)
Trends in Opioid Analgesic Abuse and Mortality in the United States
Postmarketing surveillance indicates that the diversion and abuse of prescription opioid medications increased between 2002 and 2010 and plateaued or decreased between 2011 and 2013. These findings suggest that the United States may be making progress in controlling the abuse of opioid analgesics (NEJM)

Drugs and Development: The Great Disconnect
This report argues that ‘drugs’ are a development issue and must be recognised as such by development agencies (GDPO)
Safety and effectiveness of dipeptidyl peptidase-4 inhibitors versus intermediate-acting insulin or placebo for patients with type 2 diabetes failing two oral antihyperglycaemic agents
DPP-4 inhibitors were superior to placebo in reducing HbA1c levels in adults with T2DM taking at least two oral agents. Compared with placebo, no safety signals were detected with DPP-4 inhibitors and there was a reduced risk of infection. There was no significant difference in HbA1c observed between NPH and placebo or NPH and DPP-4 inhibitors (BMJ Open)
Soluble CD36 in plasma and urine: a plausible prognostic marker for diabetic nephropathy
Our study implies a plausible prognostic/adjuvant biomarker role of soluble CD36 for diabetic nephropathy (Journal of Diabetes and Its Complications)
Effect comparison of metformin with insulin treatment for gestational diabetes: a meta-analysis based on RCTs
Metformin can significantly reduce several adverse maternal and neonatal outcomes including PIH rate, incidence of hypoglycemia and NICU, thus it may be an effective and safe alternative or additional treatment to insulin for GDM women (Archives of Gynecology and Obstetrics)
Cord Serum Dipeptidyl-Peptidase 4 Activity in Gestational Diabetes
Decreased cord serum DPP4 activity in gestational diabetes mellitus might be the result of an adaptive fetal response or an early dysregulation in the entero-insular axis with consequences beyond the incretin system. Cord plasma GLP-1 levels may reflect the missing oral intake with a limited glucose sensing of L-Cells via the circulation in fetal life (European Journal of Clinical Investigation)

Hard EdgesMapping severe and multiple disadvantage
Most of us understand that people who are homeless, or offenders or drug misusers must also face a wider set of challenges. It is hard to imagine a person who has fallen into a hard drug problem, for example, who isn’t dealing with early problems stemming from childhood or who isn’t facing a new set of problems as a result of their drug taking (LankellyChase Foundation)

The latest Street Drug Survey reveals the ever more diverse range of illegal and legal drugs being used by vulnerable groups in society, with pregabalin, gabapentin and synthetic cannabinoid use of particular concern. Interviews with frontline workers also highlighted fears that drug-related deaths may continue to rise as street drug purities increase (DrugScope)
How do drug market changes affect characteristics of injecting initiation and subsequent patterns of drug use? Findings from a cohort of regular heroin and methamphetamine injectors in Melbourne, Australia
The drug market period in which injecting initiation occurred influenced the first drug injected and influenced some aspects of subsequent drug use. In the context of highly dynamic drug markets in which polydrug use is common there is a need for broad harm reduction and drug treatment services which are flexible and responsive to changing patterns of drug use (Drug Policy)
